Day 1: Making the Decision

This education session is facilitated by the Dementia Society of Ottawa and Renfrew County. The session is intended to guide caregivers through key aspects of the decision-making process as they consider whether or not to transition their loved one to long-term care. The session addresses the various emotions caregivers may experience as they consider their needs, their loved one's needs, and their options. 

Day 2: Understanding and Accessing Home and Community Care

This session is facilitated by the care-coordinators from Home and Community Care (Champlain Local Health Integration Network). The goal of this session is to provide caregivers with information on care services, including: Home care and long-term care, the applications processes; and the eligibility criteria for these services.
